INSTRUCTIONS ON HOW TO DO IT:

1. Download the My MultiSport app on your mobile in the App Store or Google Store / Play Store (click / scan and download).

2. Open the app and follow the instructions. Select your country and then press continue.

3. Agree to the terms of use.

4. The app will ask you if you want to create a virtual card, confirm and get started. (You must physically have your plastic card with you or have your card’s barcode available during the process of switching from a plastic MultiSport card to a MultiSport virtual card)

5.  Log in / register for a My MultiSport account.

6. Add your phone number and confirm it with the code that is sent to you via SMS.

7. Add a photo (important so the gym can identify you when you enter). It is important that your photo is clearly visible in the facial area.

8.  You have your virtual card enabled, just click in the app to generate the QR code.

DONE! ENJOY THE EXPERIENCE

Once you activate your virtual card, your plastic card will no longer be active – it will not work. You can only ever use one type of card – virtual OR plastic.

Terms and Conditions for the Use of the MultiSport Basic Card

1. These terms and conditions govern the provision of services within the MultiSport program and their use through the MultiSport Basic card, issued by the program provider, Benefit Systems Slovakia s. r. o. (hereinafter referred to as the “provider” or “Benefit Systems Slovakia”).

2. The card may be issued in physical form (hereinafter referred to as the “physical card”) or in virtual form throughthe MultiSport mobile application (hereinafter referred to as the “virtual card”) (hereinafter jointly or individually referred to as the “card”).

The virtual card is also governed by separate terms and conditions available in the MultiSport mobile application under the title “Terms of Use of the MultiSport Application and the Virtual Card”, which shall prevail over these terms and conditions in the event of any conflict (hereinafter referred to as the “Special Terms for the Virtual Card”).

Partner sports facilities marked “Virtual Card Only” accept exclusively the virtual card; the physical card is not accepted for entry.

3. Entry: The MultiSport Basic card may be used for a total of 10 entries per month and a maximum of 1 entry per day to selected sports facilities in the Slovak Republic, in accordance with these terms and conditions and subject to compliance with the conditions of the respective facilities.

When using the MultiSport virtual card in the Czech Republic, prior verification of the cardholder’s identity in a partner facility within the territory of the Slovak Republic in accordance with point 14 of these Terms and Conditions is required.

4. Activities: The MultiSport Basic card entitles its holder to use the following groups of sports activities within the Slovak Republic:

a. Gyms

b. Swimming pools and water sports, limited to the following activities: swimming pools and outdoor summer swimming pools.

5. The MultiSport Basic card may not be used for any other groups of activities or activities that do not meet the conditions defined in these Terms and Conditions.

Issuance of the card

6. The card may be obtained through an employer, based on the conditions agreed in a contract concluded with Benefit Systems Slovakia s. r. o. (hereinafter referred to as the “contract”).

7. An employee is entitled to one card, under the conditions specified in the contract. With respect to the MultiSport Basic card, it is not possible to request the issuance of a companion card or a children’s MultiSport Basic card, i.e. the Basic card does not allow the issuance of associated cards.

8. If a holder has been issued a MultiSport Basic card, they cannot simultaneously hold a MultiSport Classic card. If such a situation is identified, Benefit Systems Slovakia s. r. o. is entitled to immediately cancel one of the cards.

9. SWAP: The card holder has the right to switch (SWAP) from the MultiSport Basic card to the MultiSport Classic card, and vice versa.

    This exchange (SWAP) may be initiated by the holder only through the holder’s employer, with effect from the beginning of the calendar month following the submission of the change request, provided that the deadlines for ordering new cards and cancelling active cards are complied with in accordance with the contractual terms agreed between the card holder’s employer and Benefit Systems Slovakia s. r. o..

    10. Based on an agreement with the employer, Benefit Systems Slovakia is entitled to determine that the card holder may use the card only in its virtual form; in such a case, the card holder’s right to choose to use the card in physical form does not apply.

    Terms and conditions for using the card in partner facilities

    11. The Card is issued to a specific first name and surname, is non-transferable and must not be made available to any other persons. It is intended exclusively for the personal use of MultiSport programme services by the cardholder. The Card may not be used at any Partner Facility to which the cardholder has a relationship that could result in a conflict of interest or undue advantage, in particular a family, employment or similar relationship, functional, ownership or other relationship, whether direct or indirect. The Card may also not be used in connection with the performance of the cardholder’s profession or business activities.

    12. Before using the service, the card holder is required to present the card and a personal identification document, such as an ID card, driver’s license, or, in the case of a child, a school ID or health insurance card. Presenting the card during or after the use of the service is prohibited. An authorized employee of the partner sports facility is entitled to request that the holder present an identification document for inspection and is obliged to verify that the person is the authorized card holder. The holder of a virtual card is generally required to present an identification document only upon its first use, when changing a mobile phone, or if the partner facility employee is prompted by the system used to scan the cards.

    13.  If the partner sports facility does not have a card reader or if the reader is temporarily out of service, the card holder is required to record their visit and confirm it with their signature in a special form available at the facility’s reception.

    14. The partner sports facility is entitled to retain the plastic card if it determines that the name and surname on the card do not match the details on the presented identification document, or if it has other reasonable grounds to suspect that the card has been or is being misused or used by an unauthorized person.

    15. The card holder is required to always keep the card with them during their visit to the facility, i.e. in their possession. It is prohibited to leave the card, for example, at the reception (including as a deposit for a locker key or other equipment).

    16. It is prohibited to use the card after the end of its validity period.

    17.  After the plastic card’s validity period has expired, the card holder is obliged to return it to the employer.

    18. It is prohibited to make any changes to the appearance of the card.

    19. It is prohibited to make any photocopies, scans, or photographs of the card.

    20. It is prohibited to use the card in any manner other than as specified in these terms and for any purpose other than that for which it is intended under these terms. In addition to the above, it is particularly prohibited to trade in the cards in any way, distribute them, sell them, provide them to or lend them to third parties without the prior written consent of Benefit Systems Slovakia, as well as to handle the cards in any other way for the purpose of obtaining unjustified enrichment.

    21. The card holder is required to comply with the operating rules and other conditions of the partner sports facility.

    22. Some partner sports facilities may require the fulfilment of additional conditions to use their services, in particular initial registration, including any registration fee, or booking through a reservation system. If a partner sports facility sets such conditions, the card holder is required to fulfil them as a condition for being granted entry.

    23. The partner sports facility has the right, without any compensation, to refuse entry using the card if the card holder fails to comply with these terms (for example, if they refuse to present a photo identification document) or with the conditions of the respective sports facility, if there is reasonable suspicion that the card would be used in breach of these terms, or if the facility or activity is currently fully booked.

    24. Notwithstanding the above rules, card holders are advised to contact the respective facility in advance to verify the conditions for using its services.

    Loss, destruction or theft of the card

    25. The card holder is obliged to report the loss, destruction, or theft of the plastic card without undue delay, no later than within 24 hours, to info@multi-sport.sk and at the same time to their employer, in order for the card to be blocked. If a card that has been reported as lost or stolen is subsequently found, the card holder is obliged to send it to Benefit Systems Slovakia.

    Suspension and cancellation of the card

    26. The card holder may request the suspension of the card for an indefinite period solely for serious health reasons. The request for suspension must be submitted through the employer no later than the order deadline for the following month. In such a case, the validity of the card will be suspended from the first day of the following calendar month.

    27. The card holder may cancel the card with effect as of the end of a calendar month, usually based on a request submitted through the employer no later than the order deadline for the following calendar month. In the event of card cancellation, the card holder may order the card again no earlier than after 6 calendar months, unless the agreement provides otherwise.

    28. Benefit Systems Slovakia is entitled, through its authorised personnel (hereinafter referred to as the “Inspector”), to monitor compliance with these Terms and Conditions also on-site at Partner Facilities. For this purpose, the Inspector is authorised to ask persons present at a Partner Facility whether they are holders of a MultiSport card. Persons who have used the Card during the relevant visit are obliged, upon request, to confirm this fact and present the Card together with an identity document for inspection for the purpose of identity verification, and to provide the requested cooperation. Refusal to provide cooperation shall constitute a breach of these Terms and Conditions with the consequences set out in Clauses 29 and 30 hereof.

    Consequences of breach of the Terms and Conditions

    29. The card is the property of Benefit Systems Slovakia s.r.o.. Benefit Systems Slovakia has the right to withdraw or block the card, in particular in the event of a breach of these terms by the card holder, or in other cases agreed in the contract, especially due to non-payment of the service fee by the employer.

    30. If the card holder breaches the conditions set out in points 11, 12, 15, 16, 19 or 20 of these terms, Benefit Systems Slovakia, in addition to the rights specified in  point 29, is entitled to refuse to issue a card to that person in the future and/or to require the card holder to pay a penalty of €15 for each breach and/or seek compensation for the actual damage incurred as a result of a breach of these Terms and Conditions, including demonstrable costs associated with the investigation of such breach.. Benefit Systems Slovakia is entitled to inform the card holder’s employer, or the employer of the employee card holder in the case of affiliated cards, about the breach of obligations. This does not affect the right of the partner sports facility to refuse entry.

    Liability for damage

    31. Benefit Systems Slovakia shall not be liable, to the extent permitted by applicable law,for damage incurred by the card holder in connection with the use of the services of the MultiSport program or arising where it is not possible to use the services of the MultiSport program in accordance with these terms, nor for any damage resulting from a breach of these terms by the card holder. Liability for damage shall rest with the person who caused it by their actions.

    Binding nature of these Terms and Conditions

    32. By ordering, collecting, or using the card, the card holder declares that they have familiarized themselves with the current version of the card usage terms, are aware of their obligation to comply with them, and acknowledge the consequences resulting from their breach.

    33. Benefit Systems Slovakia reserves the right to amend or supplement these Card Terms and Conditions at any time. The current version of the Terms and Conditions is published on the company’s website. Any amendments shall become effective on the date specified in the updated version of the Terms and Conditions, but not earlier than on the date of their publication. Continued use of the Card after the effective date of such amendments shall be deemed acknowledgement and acceptance of the updated Terms and Conditions.

    34. The latest update of these card usage terms was made on 01.12.2025.
